On Your First Appointment
Please bring your:
Driver’s license or legal photo ID
COVID-19 Vaccination Card
Cash, check, or credit card to cover any visit fee/co-pay.
Payment is required at the time of service.
Return Your Completed New Patient Forms
Please download, complete, and return the NEW PATIENT FORMS.pdf at least
48 hours prior to your first appointment.
Due to the insurance verification process,
we may have to reschedule your appointment
if we do not receive the forms 48 hours in advance of your scheduled appointment.
Your insurance company determines the portion of your care that is your responsibility. Due to the number of different health insurance providers and plans, each person's health care coverage fee for service varies (due to co-pays, annual deductibles, insurance fee schedules).
PLEASE contact your insurance to determine your benefits and what you are expected to pay each visit.
Payment for service of copay/visit is due at each appointment.
For your convenience, we accept:
Cash, Checks, Visa, MasterCard, American Express, and Discover.
Our goal is to provide quality health care to all our patients in a timely manner.
Please understand that appointment times are limited. When you book your appointment, you are holding space on our calendar that is no longer available to our other patients.
If you must cancel your appointment, we respectfully request a minimum of 24-hours notice. Missed appointments, or appointments canceled without 24-hours notice, will incur a fee of $75.
